Payroll

The Payroll site allows our employees the ability to view, print, and save their payroll statements and Form W-2. Both current and past pay information will be accessible 24/7 for a period of 7 years from our start date of July 1, 2011.

Benefits

Community Coffee is dedicated to providing a comprehensive Benefits package for our employees. The links below will take you to websites relating to Community Coffee's Online Benefit Plan Enrollment, Health, Dental and 401(k) Plans. If you have questions regarding your Benefits, please contact Linda Donnelly, Benefits Manager, or Emily Mungall, Benefits Assistant, in the Human Resources Department.

Online Enrollment

All Benefit Plan Enrollments are processed online. Employees may access the Online Benefit Enrollment website by clicking the OLE button below. Access into the website is secure and requires a username, company identifier and password. If you do not know this information, contact Emily Mungall or Linda Donnelly in the Human Resources Department

Health Plan

United Healthcare (UHC) is our third-party administrator of Community Coffee's Health and Flexible Spending Account Plans.

If you are a Plan Participant, you may access the UHC website by clicking the My UHC button below. Access into the website is secure and requires a username and password. First-time users must initially register and select a username and password of your choice. Health Plan Participants and covered spouses are required to take a mandatory health assessment.

The website provides account and/or clains activity, claim forms, UHC provider lists, benefits and coverage, personal health record, health and wellness tools and resources, estimate health and/or pharmacy costs. Also, you may print a temporary ID card. Additionally this is the site to initially take the Mandatory Health Risk Assessment and/or update it each year.

Dental Plan

MetLife Dental is the insurance carrier of Community Coffee's Dental Plan.

If you are a Plan Participant, you may access the MetLife website by clicking the MetLife Dental button below. Access into the website is secure and requires a username and password. First-time users must initially register and select a username and password of your choice.

The website provides account and/or claims activity, claim forms, MetLife provider lists, and benefits and coverage. Also, you may print an I.D. card.

Vision Plan

United Healthcare (UHC) is the insurance carrier of Community Coffee's Vision Plan.

If you are a Plan Participate, you may access the UHC Vision website button below. Access into the website is secure and requires Subscriber's Unique Identification Number (Covered Employee's Social Security Number), Subscriber's Last Name (Covered Employee's Last Name), and Patient's Date of Birth (Covered Employee's Date of Birth). NOTE: Dependents (Spouse/Child(ren)) must provide the covered Employee's information, not their own.

Example:	John Smith is the Covered Employee.
	Jane Smith is Covered Spouse.
	James Smith is Covered Child.
	Jane and James will input John’s Unique Identification Number, Last Name, and Patient's Date of Birth.

The website provides account and/or claims activity, claim forms, UHC provider lists, and benefits coverage. Also, you may print an I.D. card.

401(k) Plan

Fidelity Investments is our 401(k) Retirement Savings Plan Recordkeeper. Eligible participants may access the Fidelity website by clicking the Fidelity Investments button below. Access into the website is secure and requires a username and password. First-time users must initially register and select a username and password of your choice. This is the website to enroll and/or make changes to your Community Coffee 401(k) Retirement Savings Plan Account. The website also provides account balances, investment fund information, and tools and resources for managing your savings plan.

The CCC Library

The CCC Library is an online resource for Community Coffee's employees that provides opportunities for learning and personal development. To access the site, you will need to have the username and password that are provided in the CCC Library section under the Education and Development Department on Javanet. (If you do not have access to Javanet, please contact your Supervisor to obtain the username and password.)

The Company Store

The Company Store is your source for Community Coffee's Uniform/Team Wear. The access to this site is managed by our Purchasing Department. If you have any trouble accessing the site, please contact Purchasing at 1-800-688-0990.
